CHANGE MY PERSONAL DETAILS?
If you
change address please advise reception. If you move outside of the designated
practice area you will be requested to register with an alternative practice
as we will not be able to provide you with a full service, particularly in respect
to home visits. It is very difficult and time-consuming for our doctors to travel
outside of the practice area, as it is already a very large geographical area. We would also like to know if you change your name (eg upon marriage) or your telephone number.
